Enrollment Process

Congratulation on your acceptance to SHDHS! We are thrilled to welcome you to our community of students, educators, alumni, and lifelong friends. Please follow the steps below to complete your enrollment and secure your spot as a new Crusader.

New Student(s) Enrollment Process

Please login to the SHDHS Enrollment Portal and complete the following steps.

  1. Enrollment Registration and fee
  2. Schedule Counselor Meeting
  3. Complete FACTS Acknowledgement Form (Every parent must complete the required information for their family account through FACTS Family Portal, our tuition payment provider.)
  4. Non-Aspirin Medication Form
  5. Media/Directory Publication Consent Form
  6. SHDHS Responsible Use Policy Form
  7. SHDHS Handbook Acceptance Form
  8. Chromebook Purchase Form

*Enrollment at St. Henry District High School is contingent upon a student’s remaining in good academic and behavioral standing in their current school and completing the 8th grade. Enrollment involves the completion of all forms and fees.

Current SHDHS Student(s) Re-Enrollment Process

  1. Go to SHDHS Enrollment Portal
    • SHDHS is using a new enrollment/re-enrollment application. If this is your first time here, please create a new account. Once you have completed your account registration, you will access the SHDHS Enrollment Dashboard. Please use the Re-Enrollment option for current students.
  2. Select Re-Enrollment Option
  3. Complete FACTS Acknowledgement Form (Every parent must complete the required information for their family account through FACTS Family Portal, our tuition payment provider)
  4. Non-Aspirin Medication Form
  5. Media/Directory Publication Consent Form
  6. SHDHS Responsible Use Policy Form
